General Service Description         

Te Puawaitanga ki Otautahi Trust (TPKOT) evolved out of the Otāutahi branch of Māori Women’s Welfare League (OMWWL), which was established in 1952. TPKOT is a leading Kaupapa Māori provider within the Canterbury area, delivering community-based services to support Maori, Women and their Whānau. TPKOT provides predominantly home-based services aimed specifically at addressing health and other inequities experienced by under-resourced groups within the community. Services are based on Kaupapa Maori ideals to work in a holistic manner to enhance the health and well-being of the community we serve.

Our teams aim to work alongside you and your Whanau to find out how we can support your health and wellbeing. We provide information, support and advocacy at regular follows-ups. You determine your needs and we aim to help you achieve them.

Tamariki Ora / Well Child Service          

Our Tamariki Ora team provides

well child checks, education,

support and follow-up for parents and

caregivers of predominately

Maori babies from 0 - 5 years of ages.

Our Registered Nurses provide:

  • Well Child Checks in line with theNational Well Child Framework
  • Additional advice and support towhanau about breastfeeding, first foods,sleep issues, child development and other parenting issues

Our Kaiwhakapuawai provide:

  • Additional advice and support to whanau about breastfeeding, first foods,sleep issues, child developmentand other parenting issues
  • Support and referral to health, social,educational, justice and welfare agencies
  • A diverse knowledge of communitynetworks / support  development of Te Ao Maori

Outreach Immunisation Service

Tamariki Maori, and other priority groups 

aged 0 - 6 years old that are behind on

the Childhood Immunisation Schedule.

After receiving the referral our Registered

Nurse Vaccinator and Kaiwhakapuawai

can offer vaccination in the setting of your

choice.

 We provide:

 

  • Up to date information on choices about immunisation for you and your Whanau
  • Facilitation of immunisation in the place you are most comfortable
  • Promote public awareness of the devastating effects of MenzB

Rapuora Maori Mobile Nurses Service

The Rapuora Maori Mobile Nurse team

provide a Home/Marae based service to

those in the community with chronic health disorders. The service is available to

Whanau who have or who are at risk

of heart disease, lung disease or diabetes.

Our aim is to work alongside clients and

their whanau to find out how we can best support the management and

improvement of their health and wellbeing.

Our service offers:

  • Health Checks
  • Advice, education and support with client health concerns
  • Advocacy and cultural support at GP, Specialist or hospital visits
  • Help with medical questions

Practical management advice and linkages i.e weight loss, smoking cessation support

Canterbury Breastfeeding Advocacy Service

Our breastfeedingadvocacy service works with a range of community groups, health, social and education services, and businesses to work towards a breastfeeding friendly Canterbury. A peer to peer support programme has also been established to assist in removing barriers to and supporting mothers who are breastfeeding.

Gilberthorpes Community Edible Garden is to provide an opportunity for members of the community including the local Te Kohanga Reo and primary school children to learn about the science, social and health benefits of growing produce.

Parents As First Teachers - PAFT

 

Ko Nga Matua Hei Kaiako Tuatahi

PAFT is based on the philosophy that parents are their children’s first and most important teachers, and offers support and guidance in this role. We provide practical and effective support for children aged 0 - 3 years old during monthly home based visits

 Parent Educators can help you :

  • Understand how your child grows and develops
  • Know how to encourage your child’s growth and development
  • Gain confidence in your own parenting skills
  • Get the support and professional assistance you need for your child’s health and development
Provide opportunities to meet other parents in various small group or large whanau events

Promoting Participation in Early Childhood Education

Our PPP worker works with Whanau to identify and access suitable early childhood education for Tamariki. We will support you to centre visits to help you determine the best option for your Tamaiti.

Community Support + Development for Maori in Hornby rohe 

Our community development worker, works with the Maori community, other  community groups, schools, preschools and services to identify and address the needs of Maori in Hornby.
